Big News: ACC Puts Inflammation Center Stage

In a landmark 2025 scientific statement, the American College of Cardiology (ACC) formally acknowledges what functional medicine has emphasized for years: chronic, low-grade inflammation is a key driver of cardiovascular disease (CVD). The report elevates high-sensitivity C-reactive protein (hs-CRP) to a central role in both risk assessment and management, marking a major shift in conventional cardiology.
Highlights from the 2025 ACC Statement

Universal screening: The ACC recommends measuring hs-CRP alongside cholesterol in both primary and secondary prevention, calling it a “major clinical opportunity.”

Predictive strength: hs-CRP is as powerful—or more predictive—than LDL cholesterol for future cardiovascular events, even among patients with normal lipid levels.

Risk categories:

<1 mg/L: Low risk
-
1–3 mg/L: Average risk
-
>3 mg/L: High risk (recheck if >10 mg/L and the patient may be acutely ill)

Primary prevention: A persistently elevated hs-CRP (>3 mg/L) should prompt attention to lifestyle interventions such as diet, exercise, smoking cessation, and weight management. The ACC also notes that statin therapy may be beneficial even when LDL is normal in this setting.

Secondary prevention: Among individuals with established CVD, hs-CRP >2 mg/L indicates “residual inflammatory risk.” In these patients, low-dose colchicine (0.5 mg daily), now FDA-approved for reducing recurrent cardiovascular events, can be considered alongside standard lipid-lowering therapy.
Takeaway:
The ACC’s endorsement of hs-CRP testing signals a paradigm shift: inflammation is no longer a peripheral concept—it’s now recognized as a clinically actionable target in cardiovascular prevention and management.
